This privacy policy defines the principles of processing personal data by Stomatologia Chrobrego dental offices, located at ul. Chrobrego 4A/1 in Stargard, operated by Solidental Spółka z o.o., with headquarters at ul. Twarda 18, 00-105 Warsaw, NIP: 5252838484, registered in the District Court for the Capital City of Warsaw, XII Commercial Division of the National Court Register.

3. Purpose of Data Processing

Your personal data is processed for the purpose of:

  • registering and handling patient visits,
  • maintaining medical records,
  • issuing invoices and billing documents,
  • responding to inquiries (via phone, email, or form),
  • fulfilling legal obligations,
  • ensuring continuity of care and communication with patients.

Note: In order to attend a visit, providing a PESEL number is mandatory. Lack of this information makes it impossible to provide medical services in accordance with applicable law.

4. Legal Basis for Processing

  • Art. 6(1)(b) GDPR – performance of a contract,
  • Art. 6(1)(c) GDPR – legal obligation,
  • Art. 9(2)(h) GDPR – health care and medical diagnosis.

5. Data Recipients

Data may be transferred to:

  • entities processing data on our behalf (e.g., laboratories, prosthetic labs, IT service providers),
  • entities authorized by law (e.g., NFZ, Tax Offices, regulatory authorities).

6. Patient Rights

Every patient has the right to:

  • access data,
  • rectify data,
  • restrict processing,
  • object,
  • lodge a complaint with the Data Protection Authority.

7. Data Retention Period

  • Medical records: 20 years (as per the Patient Rights Act),
  • Accounting records: 5 years,
  • Contact data: until consent is withdrawn or claims expire.

8. Voluntary Data Provision

Providing personal data is voluntary but necessary for the provision of medical services.

9. Cookies and Technical Data

The website may use cookies to ensure proper operation, analytics, and integration with external services (such as Google Maps, YouTube videos, or contact forms).

According to the law, the user must be informed about cookies and give consent to their use – except for those essential for website functionality.

On the first visit, a banner appears with cookie information and consent settings. Users can change cookie preferences in their browser at any time.

10. Policy Changes

We reserve the right to update this privacy policy. The current version is always available on the website.

11. Video Surveillance

To ensure the safety of patients, staff, and property, Stomatologia Chrobrego uses video surveillance on its premises.

Access to the recordings is granted only to authorized personnel. In justified cases, the recordings may be made available to law enforcement authorities, such as the Police, in accordance with applicable legal regulations.

The monitoring is conducted in a manner that does not infringe on the dignity and privacy of patients. Its purpose is to ensure the safety of individuals and property.

Cameras are installed in common areas – such as hallways, treatment rooms (dental offices), and the reception area. The surveillance does not cover toilets or staff rooms.

  • The legal basis for processing data from video surveillance is Article 6(1)(f) of the GDPR (legitimate interest of the data controller).
  • Recordings from the surveillance system are stored for a maximum of 14 days, after which they are automatically deleted, unless they are evidence in legal proceedings.
  • Persons recorded by the surveillance system have the right to access the recordings and to object to the processing of their data.
